What Note Types Are Available?

Echo-Health supports a wide range of clinical note types to suit different specialities, documentation styles, and jurisdictional needs in Australia and New Zealand. Each note type has been designed in collaboration with clinicians and mapped to relevant specialty areas to streamline documentation, align with best practice, and support downstream analytics.

🌍 Jurisdiction-Specific Support

All note types are jurisdiction-tagged (e.g., AU, NZ) to reflect regional documentation preferences, healthcare workflows, and coding systems. This allows healthcare providers to select templates that align with local clinical expectations.

Below is a sample of supported note types:

  • SOAP
    Standard structure: Subjective, Objective, Assessment, and Plan. Common in general practice and multidisciplinary care.

  • SOAP (Assessment & Plan merged)
    Combines Assessment and Plan into a single section. Useful for faster documentation while retaining structure.

  • SOAP (Nursing)
    Tailored for nursing practice with clear delineation of subjective and objective data, and care planning.

  • APSO
    Prioritises the clinician’s analysis and plan (Assessment & Plan) before Subjective and Objective details. Favoured in fast-paced consults.

  • SO (Nursing)
    A simplified structure with just Subjective and Objective sections, typically used in nursing shift notes or brief updates.

  • HOPC
    Commonly used in Australian primary care. Captures the History of Presenting Complaint along with examination, impression, and management.

  • Chief Complaint Format
    Focused format for short consults. Includes presenting problem, brief history, findings, and plan.

  • Multiple Presenting Problems
    Designed for complex visits with multiple concerns. Breaks documentation into multiple sub-problems.

  • Comprehensive Clinical Note
    A full-format template for holistic patient documentation. Includes past medical and surgical history, medications, social history, assessment, and plan.

  • Specialty-Specific Notes
    Tailored formats are available for:
